FiveM Medical System β€’ Interactive DocPad β€’ Player EMS β€’ NPC Ambulance β€’ NPC Medical Missions

LRRP Death & Medical Response is a complete FiveM death, EMS dispatch and paramedic gameplay system designed to make the medic job much more interactive.

Instead of simply dying and waiting for a revive, players automatically create a medical emergency. Every on-duty medic is notified, but the first medic to accept the call gets priority and receives the GPS waypoint.

If nobody responds, a configurable NPC paramedic arrives in an ambulance, approaches the patient, performs an interactive medical assessment and revives them.

And when the server is quiet? Medics can receive procedural NPC medical missions so there is still something to do. 🩺

🚨 Player Emergency System

πŸ’€ Automatic medical call when a player dies
πŸš‘ All on-duty medics are alerted
βœ… First medic to accept claims the emergency
πŸ“ Automatic GPS waypoint on acceptance
πŸ—ΊοΈ SET WAYPOINT available directly inside /docpad
πŸ”’ Prevents multiple doctors racing to the same patient
πŸ‘¨β€βš•οΈ Player medics always have priority over NPC calls
❀️ Revive directly through DocPad
πŸŽ’ No revive item required

πŸ““ Interactive Medic DocPad

Use:

/docpad

The DocPad acts as the medic's central working interface.

Doctors can:

🟒 Go On/Off Duty
🚨 View active emergencies
βœ… Accept / decline calls
πŸ“ Set or refresh patient waypoints
πŸ‘€ View the assigned patient
🩺 Perform patient assessments
❀️ Check pulse
🫁 Check breathing
🩸 Examine injuries
πŸ’¬ Talk to patients
🩹 Control bleeding
❀️ Perform CPR
⚑ Defibrillate
πŸ₯ Stabilise patients
πŸš‘ Handle NPC patient transport
🏨 Set hospital waypoints
πŸ“Š Track NPC mission statistics
❌ Release an accepted emergency

Only the doctor who successfully accepts a call receives control of that emergency.

πŸš‘ NPC Ambulance Response

No player medic available?

The automated medical service can take over.

The system will:

πŸš‘ Spawn a configurable ambulance
πŸ‘¨β€βš•οΈ Spawn a configurable paramedic
🚨 Drive to the patient with emergency response
🚢 Exit the ambulance and approach the patient
πŸ’¬ Start the medical dialogue system
❓ Perform a multi-question assessment
🩺 Treat the patient
❀️ Revive the player
🧹 Clean up the NPC and vehicle afterwards

A player medic can still take priority while the NPC ambulance is travelling.

Once NPC treatment has started, the emergency is locked to prevent conflicting revives.

πŸ’¬ Interactive Medical Dialogue

NPC treatment isn't just an instant revive.

The medical dialogue system includes:

πŸ—£οΈ 20 opening dialogue variations
❓ 32 medical questions
πŸ”΅ 96 selectable patient answers / responses
πŸ’¬ 20 treatment/closing lines
🎲 Randomised questions during treatment

The same dialogue presentation is also used during NPC patient missions.

🚨 NPC Medical Missions

Quiet server?

On-duty medics can receive procedural civilian emergencies.

By default the system generates 1–3 random NPC medical calls during each 90-minute window.

Everything is configurable:

Config.NPCMissions.Enabled = true
Config.NPCMissions.WindowMinutes = 90
Config.NPCMissions.MinCallsPerWindow = 1
Config.NPCMissions.MaxCallsPerWindow = 3

Calls are distributed randomly throughout the window rather than appearing together.

Possible emergencies include:

πŸš— Traffic accidents
πŸ”« Gunshot victims
πŸ”ͺ Stabbing victims
❀️ Cardiac emergencies
😡 Collapsed/unconscious civilians
πŸ’Š Suspected overdoses
πŸ—οΈ Falls and workplace accidents
🌊 Drowning incidents

Doctors must investigate the patient rather than simply pressing Revive.

🩺 NPC Patient Assessment

Depending on the call, medics can:

πŸ’¬ Talk to the patient
❀️ Check their pulse
🫁 Check breathing
🩸 Examine injuries
🩹 Control bleeding
❀️ Perform CPR
⚑ Use the defibrillator
πŸ₯ Stabilise the patient

Incorrect treatment doesn't instantly kill the NPC β€” it simply won't progress the correct treatment path.

Serious patients can require transport:

Stabilise β†’ Load Patient β†’ Ambulance Transport β†’ Hospital Waypoint β†’ Drop Off

πŸ₯ Default hospital locations are included and additional hospitals can be configured.

🚨 Player Calls ALWAYS Take Priority

The medical priority system is:

1️⃣ Real player emergency
2️⃣ Player medic response / backup
3️⃣ NPC medical mission

A waiting real-player emergency prevents new NPC missions from taking priority.

If a doctor is working an NPC mission when a real player needs help, the real emergency is surfaced as the priority call.

πŸ”§ Configurable Ambulance & Paramedic

Don't want to use the default GTA ambulance or paramedic?

Change them.

Config.NPC.VehicleModel = 'ambulance'
Config.NPC.PedModel = 's_m_m_paramedic_01'

βœ… GTA vehicles
βœ… Custom streamed ambulances
βœ… Addon vehicles
βœ… Custom medic peds

Mission locations, hospitals, NPC models, timers, rewards and other settings are configurable.

βš™οΈ Framework Support

Supported:

βœ… QBCore
βœ… Qbox
βœ… ESX Legacy
βœ… Standalone
βœ… Custom Framework Bridge

Automatic detection is available:

Config.Framework = 'auto'

The resource checks for supported frameworks and can fall back to its standalone medical-job implementation.

πŸ‘¨β€βš•οΈ Built-In Standalone Medic Job

Don't use QBCore, Qbox or ESX?

No problem.

Standalone servers can use LRRP's built-in persistent medic authorisation and duty system.

Admin commands include:

/lrrpmedic add [serverId]
/lrrpmedic remove [serverId]
/lrrpmedic list

ACE permissions are also supported.

πŸŽ’ No Revive Items Required

Doctors do not require a medkit, first-aid kit, syringe or other inventory item to revive a patient.

Medical access is controlled through the job/duty and emergency systems rather than inventory consumption.

πŸ†“ FREE EDITION

The Free edition contains the full gameplay system, including:

βœ… Player medic dispatch
βœ… /docpad
βœ… Interactive assessment
βœ… NPC ambulance response
βœ… NPC medical dialogue

Free Limits

πŸ‘¨β€βš•οΈ 3 player-medic accepted real-player calls globally every 24 hours

πŸš‘ 3 NPC ambulance responses to real players globally every 24 hours

These are separate server-wide/global allowances, not per-player limits.

NPC side missions do not consume these allowances.

Once the applicable Free medical service allowance has been exhausted, the player receives the remaining reset time and Premium upgrade notice before receiving a basic revive without the NPC ambulance/treatment sequence.

Counters persist through resource/server restarts.

Premium removes the Free restrictions.

♾️ Unlimited player-medic accepted emergencies
♾️ Unlimited NPC ambulance responses
🚫 No LRRP branding
πŸ““ Clean Premium DocPad
πŸš‘ Full NPC ambulance system

πŸ“¦ Installation

Current resource names:

ensure LRRP_Medic
ensure LRRP_docpad

⚠️ Do not rename the LRRP resources.

Database

βœ… No SQL required

Standalone medic assignments, statistics and Free-edition limits use persistent resource data.

πŸ§ͺ Admin Testing

NPC medical missions can be tested without waiting for the normal schedule:

/lrrpmissiontest

Requires the configured LRRP medic admin permission.

πŸ“‹ Requirements

Game: FiveM / GTA V
Manifest: cerulean
Lua: Lua 5.4
Framework: Standalone, QBCore, Qbox, ESX Legacy or Custom Bridge
SQL: None required
Inventory: No inventory dependency required
Revive Item: Not required
OneSync: Recommended for multiplayer/NPC synchronisation

⚑ Performance

The medical system is primarily event-driven. NPC ambulance and patient logic becomes active when an emergency or NPC mission is running rather than continuously spawning mission entities.

Important for the CFX post: add a real resmon/profiler measurement from your test server before publishing. CFX explicitly asks release authors to provide performance indicators; I wouldn't invent a 0.00ms figure without actually measuring the resource in FiveM.
